Pamela Yee, MD

Dr. Yee will be continuing in private practice after leaving Blum Center. Patients currently under her care will have the opportunity to continue working with her directly.

If you are working with another provider and would like to explore transferring your care to Dr. Yee, please contact us to discuss availability and transition planning.

We expect to have full details by August 1, 2026 regarding how to contact Dr. Yee directly once Blum Center closes.

Vicki Kobliner (Nutrition) and Melissa Rapoport (Health Coaching)

Both Vicki and Melissa will continue seeing patients independently after Blum Center closes.

You may continue scheduling appointments with them at Blum Center through August. During those visits, they can help guide your transition plan and discuss options for ongoing support.

Dr. Andrews and Macaulay Kerr

Dr. Andrews and Macaulay will be joining membership-based organizations after leaving Blum Center.

Patients interested in learning more are welcome to contact them in the portal for additional information.

Dr. Susan Blum

If you are one of Dr. Blum’s patients, transition planning has already been discussed with many of you during recent visits and will continue to be reviewed during upcoming appointments.

If at any time you would like additional support reviewing your wellness plan or discussing next steps, you may schedule a session with Melissa Rapoport.

Amruta Inamdar

Amruta will be transitioning to Score PT in Rye at the end of June 2026.

Patients wishing to continue physical therapy care with her may follow up directly there.

Your medical records will be securely maintained by the Blum Center for Health for ten years following closure.  We recommend requesting a copy of your medical record in order to ensure the smoothest possible transition to your future providers.

If you would like a copy of your medical records, you may contact our office at 914.652.7800 to request a copy of your medical records.

We will send another letter to let you know to whom requests for medical records should be directed after August 28, 2026. This information will also be posted on the Transition Information page on our website.

After closure:

  • instructions for requesting records will remain available on our website
  • a records custodian service will manage ongoing requests
  • detailed instructions will be shared in future communications

Because providers remain legally responsible for reviewing ordered testing, all laboratory and imaging studies must be completed before the deadlines below.

Testing Deadlines

  • Dr. Andrews’ patients: no testing available. All testing for Dr. Andrews’ patients has been completed.
  • Macaulay Kerr’s patients: testing must be completed by July 15, 2026
  • Dr Blum, Dr. Yee, Vicki, and Melissa’s patients: testing must be completed by August 1, 2026

Any incomplete testing ordered through Blum Center after these dates will need to be canceled. We apologize for the inconvenience, but this is necessary to ensure patient safety and proper medical oversight.

If you continue working with any of our providers in their future practices, they can reorder testing that was unable to be completed before closure.

We are happy to facilitate a transition of care for patients who wish to move to another provider after Blum Center closes. We are in discussions with another medical provider in Rye, New York who also provides a GLP-1 peptide services who may be available to provide services to the Blum Center’s patients.  We expect this transition to occur in September 2026, and we will continue supporting your care until then. Additional details will be shared as soon as they are finalized.  However, if you wish to transition your GLP-1 care to another medical provider, we are happy to provide your medical records to such provider upon your request.

Where medically appropriate, we will provide bridge refills for up to three months.

After that time, prescriptions will need to be managed by your new provider or primary care physician.

We encourage you to contact your provider early if you anticipate needing refills during the transition period.

Although our physical clinic will be closing, Dr. Blum will continue to maintain various aspects of the business.

This means your Fullscript account will remain active indefinitely under the existing Blum Center account, allowing continued access to supplements and dispensary pricing.  We will also be implementing a 10% patient discount.

We still have inventory of many commonly recommended supplements available in our store.

We’re offering a 20% discount on the entire store, in person and online.

We will not be replenishing inventory before closure, we recommend purchasing your supplements as soon as possible.

You may also continue ordering supplements through Fullscript.

A final note from Dr Blum

Saying goodbye to this practice and to each of you is not easy. Blum Center has always been more than a clinic. It has been a community built on curiosity, compassion, partnership, and a shared commitment to real healing. That spirit does not disappear when the doors close.

The knowledge you have gained, the changes you have made, and the healing you have experienced belong to you. I hope you carry that forward with confidence and trust in yourself.

Thank you for allowing us to be part of your journey. Caring for this community has been one of the greatest honors of my life, and I will always hold deep gratitude for the trust you placed in us.

With warmest wishes and heartfelt gratitude,

Susan Blum, MD, MPH
